That does look like it would take a lot of elective surgery to pull off
So the question is, have you got an old style butt plug with a tail on it, or one of the new fancy ones that puts the tail higher up in a realistic position? (May require some clenching to keep in place …)

The only thing I can think to do is a black bodysuit with the organs printed on. Maybe slightly glow in the dark so they stand out.

I was amazed at the Krasue that Dead by Daylight added. Sadly, in its current version it’s not a lot of fun.
