A step ladder is called a step ladder because of the steps, but here the author subverted that obvious meaning to have “step” mean that they are part of different families related by marriage, and then inserting that into the stepbrother trope I linked to above, resulting in an even more ridiculous situation than in the porn plots, since humans and ladders usually aren’t sexually interested in each other.
