This is kinda like the setup when my wife had her Cesarian. Curtain up over the waist, wide awake, anesthesiologist right behind.
She explained that it felt like rummaging around inside a purse, except she was the purse.

Sometimes full sedation is too risky, or at least not worth it for factors other than cost
True, there’s always a risk that you don’t wake up from full sedation. For most surgery you’ll also be out of the hospital quicker if you take the epidural.
And sometimes you’re somewhere along the timestream where anesthetic hasn’t been invented yet.
In the US though, it’s definitely not for the bbenefit of the patient.
I got my back teeth drilled without anesthesia. The short-term pain was better than the aching gum for two days from the shot.
